Direct primary care medical services, namely, general medical examinations, preventative health screenings, and routine physicals; health and wellness counseling, including nutrition and lifestyle counseling; health assessment services; personalized wellness programs, routines, and health maintenance scheduling; chronic disease management services for general medical conditions provided within a direct primary care model, excluding all specialized medical, therapeutic, and advanced care services, and specifically excluding: (1) Medical or therapeutic services for Alzheimer's disease, dementia, or other memory loss diseases, (2) Mental health psychotherapeutic services, including individual, group, or family therapy, (3) Substance abuse treatment, including opioid cessation counseling and medication-assisted rehabilitation of drug addicted patients, and (4) Medication management services for patients diagnosed with depression, anxiety, or schizophrenia.
