Organizing volunteer activities, namely, organizing volunteers to provide relief for overburdened caregivers that provide personal care for individuals who have been discharged from hospice care because their health has stabilized or improved and for those seriously-ill individuals who are either not yet eligible for hospice care but in need of support and for those who are terminally ill but not ready to accept end of life care and service, by assisting caregivers with running errands and performing daily tasks in the nature of chores

Educational services, namely, providing presentations dealing with elder care, end-of-life and bereavement issues; educational services, namely, volunteer training in the field of eldercare, death and dying, caregiver support, care for the dying, and related community resources

Medical services; Medical services, namely, providing healthcare services to the elderly; Hospice services; Palliative care services; Providing physical rehabilitation services and mental rehabilitation services in the nature of providing mental health services; Providing physical, occupational and speech therapy; Geriatric health care management services; Healthcare services; Health screening services in the field of blood pressure monitoring and blood glucose monitoring; Medical services, namely, providing evaluation, treatment, physician oversight, and medical direction for the care of individuals suffering from or coping with serious or life-limiting conditions; Medical end-of-life consultation services

Grief counseling and emotional support; End-of-life consultation services in field of funeral arrangements; Medical case management for individuals who have been discharged from hospice care because their health has stabilized or improved and for those seriously-ill individuals who are either not yet eligible for hospice care but in need of support or who are terminally ill but not ready to accept end of life care and service, namely, providing assistance with coordination of care of necessary medical services, including respite care and assistance with chores or errands; Providing assistance with obtaining available and needed community resources in the field of respite care for individuals who have been discharged from hospice care because their health has stabilized or improved and for those seriously-ill individuals who are either not yet eligible for hospice care but in need of support or who are terminally ill but not ready to accept end of life care and services; Providing personal support services for patients and families of patients with terminal or life threatening disorders, namely, companionship, emotional counseling and emotional support
