providing personal support services to individuals with disabilities, namely, identifying housing situations that allow the individuals to live as independently as possible; providing personal non-medical support to the elderly and/or the chronically ill and/or their families, namely, providing the individuals with companionship and personal care assistance of activities of daily living, such as bathing, grooming and personal mobility so as to maintain independence in their homes
