Providing on-line electronic bulletin boards for transmission of messages amongst computer users in the context of a website to pomote support for stroke survivors and their care partners

STROKE OF HOPE PROGRAMS/SERVICES 1; Hope Groups that provide a forum for survivors and care partners to discuss their common experiences under the guidance of a trained facilitator; Support groups are very valuable assets to assist in family recovery; 2; Community care team networks called Hope Teams available on a volunteer basis to assist survivors, families, and care partners with daily tasks within the home and guidance through the overwhelming circumstances with which they may need assistance; 3; Recovery Training offered as a supplement to physical therapy or as continued recovery after therapy is discontinued; The program is geared toward keeping the survivor actively working toward overcoming physical limitations via a regular physical routine that strengthens the body, mind and spirit; The program will be offered one on one by licensed therapists and certified personal trainers that have been carefully selected based upon their ability to effectively communicate with and encourage the survivor; Additionally, there will be professional physiologists who have earned degrees in the area of physical activity and rehabilitation to conduct group sessions giving a needed sense of belonging and family to a special group with a common purpose; Both environments will garner a community outlet for the homebound and disabled; 4; Stroke Awareness initiatives that are produced in an appealing and effective way for predominantly African-American churches, inner-city schools, health care organizations and community health fairs; 5; A volunteer-based Phone Bank to field inquiries of a general nature regarding stroke and to help direct callers to appropriate medical services for more specific questions or acute situations; 6; A web-based community information and support center, www;strokeofhope;net has monthly updated Stroke News, Survivor and Care Partner stories, message boards, and personalized communication from our Stroke of Hope Staff and Advisors
