The mark consists of a concentric shaded circles in the nature of a wheel with spokes consisting of bent lines dividing sections of the wheel. Three of the lines are solid and the others are dashed lines. The sections from the top and going clockwise contain the following wording: "Outcomes Management", "Community Education", "Primary Care Physician", "Specialist Office", "Pre- Operative Care", "Operative Care", "Hospital Care" and "Post- Operative Care". The solid lines appear between the sections "Outcomes Management" and "Community Education", "Specialist Office and "Pre- Operative Care", and "Post- Operative Care" and Outcomes Management". The inner circle contains the wording "Patient Continuum of Care". On the outside of the inner circle is a shaded semi-circle running from the 5 o'clock position on the wheel to the 11 o'clock position which contains the wording "Non-operative treatment".
