Providing a secure computer network which enables individual patients, upon their authorization, to maintain and distribute an electronic personal health and medical record for purposes of receiving emergency health and medical treatment and planning their health and medical treatment with all appropriate health and medical professionals; allowing health and medical providers to use a mechanism for receiving and transmitting protected health and medical information for the treatment and care of individual patients; providing stored information that reflects patient health and medical treatment histories; providing a mechanism for insurance companies to verify that billed services were actually delivered to individual patients; and providing a software application for physician-created electronic health and medical records, including, but not limited to, medical and dental records
